Overview

Handles CRM navbar presence heartbeats for the ERP CRM UI.

Receives lightweight authenticated heartbeats from the crm-navbar
Stimulus controller (default every 120s) at
POST /crm/navbar_presence and marks the current user as live in
CrmNavbarLiveUsers so broadcasters know they have a listener and
the periodic backstop (CrmNavbarPeriodicRefreshWorker) knows who
to refresh.

Badge updates are entirely push-driven — there is no client-side
refresh endpoint anymore. CrmNavbarBroadcaster fires Turbo Stream
replaces on every relevant write (sms / voicemail / email / search /
phone status) and CrmNavbarPeriodicRefreshWorker provides a 5-min
server-side backstop for any broadcast missed during a WebSocket
disconnect.

Heartbeats are deliberately stripped of as much before_action work
as we can drop while remaining authenticated. They run very often
(every 2 min per tab × N tabs × N employees) so even a few hundred
milliseconds of locale / impersonation / Geocoder work shows up as
material Puma thread time.
